Loss of Life Coverage UI Group Term Life Coverage is available to purchase during open enrollment or update during an IRS qualified life-style change window in the following categories. Coverage activates on the first day of the month following the date of hire unless the date of hire is the first day of the month or the extra coverage needs to be underwritten (health evidence form submitted and reviewed) by The Standard. If the date of hire is the first:day of a month, coverage begins on the date of hire. If coverage is underwritten and approved, coverage begins on the date the health evidence form was approved by The Standard.
Basic Employee Benefit ChoiceDollar funding is contributed by the University to pay for a one times salary term life benefit. Employees choose between the one times or one-half times salary benefit. If the employee selects half-time salary coverage, no other life insurance options are available. Unused ChoiceDollars are available for the employee to spend on other benefits or receive as taxable income.
Supplemental Employee Benefit If the employee elects the one times salary benefit, more salary levels of coverage may be purchased. Health evidence is required if the employee selects a total of more than three times salary coverage (one times salary basic benefits plus more than two times salary supplemental benefits). Health evidence forms are downloaded, completed and submitted to The Standard. The Standard may require up to six months to review and approve or deny the extra coverage enrollment. Cost of coverage and eligibly for extra coverage begins on the date of approval. The first two year's salary benefit coverage is effective on the first day of the month following date of hire.
Accidental Death and Dismemberment Benefit Accidental death and involuntary dismemberment coverage may be purchased at the employee's expense for various salary levels of benefit. Coverage does not require health evidence review.
Dependent benefit: Employees, at their own expense, may elect coverage for eligible family members. Different tiers of coverage are available. Spouses who are insured for more than $25,000.00 must complete and submit a health evidence form before extra coverage will be activated.
- Legal Spouse--beginning at $10,000 with increasing increments available for selection with evidence of good health
- Dependent child (6 months to 19 years, age 23 if full-time student)--available increments begin at $5,000 and increase based on employee selection
Applicability of Coverage The benefit applies during the employee's active service or leave with pay (LWP) status. For employees on leave without pay (LWOP) status, the cost of coverage is paid by the University for the first 3 months, following which, the employee may pay the cost to continue coverage for a defined period of time.
Enrollment Enrollment occurs through the employee benefit portal at www.avantserve.com
Conversion Options Employees may convert the UI group term life coverage to individual coverage within 30 days of loss of coverage. Surviving dependents of UI employees and retirees also may have conversion privileges. Questions about conversion rates and calendars may be directed to The Standard.
Designate or Change Beneficiary The employee should log into the Vandal Web employee portal to designate or change beneficiaries. Beneficiaries may be updated at any time. www.vandalweb.uidaho.edu
Cost of Coverage The University pays the cost of coverage unless an employee is on leave without pay (LWOP) for more than 3 months.
